Medical Form: PDF format. Both sides (or 2 pages) must be completely filled out, and the 2nd page (or back) must be stamped by a doctor and dated after January 1st 2008. Items needed for registration: -Football players (must be there to be weighed) -Original & copy of birth certificate -Picture (face only small size) -Medical form completed and stamped dated after Jan 1st 2008 -2 Checks for payments Away game directions are available under Message Center Remember all dates & times are subject to change, so please check with the coaches.
